Combining heroin and cocaine, a practice often referred to as “speedballing,” poses significant risks and dangers to users. This lethal combination involves the simultaneous injection or ingestion of both substances, producing a unique and highly dangerous set of effects on the body. Carriers of the 118G allele show an elevated sensitivity to pain and reduced analgesicresponse to opioids. Homozygotes for the 118G allele requested higher doses of oral morphine intreatment for cancer pain. Results of several studies suggest that the effect of the 118G allelemay vary among different opioids, different routes of drug administration, or different painetiologies, as recently reviewed (107). According to the2011 Monitoring the Future report, 1.2% of high school students in the USAreported lifetime use of heroin (43).

In the case of cocaine, for example, a mismatch would occur when this drug, which produces arousal and sympathetic activation (Billman 1995; Sofuoglu and Sewell 2009), is taken in a quiet domestic environment. The opposite would occur when heroin, which produces sedation and parasympathomimetic effects (Haddad and Lasala 1987; Thornhill et al. 1989), is taken outside the home, that is in exciting, potentially dangerous contexts. The findings reported here have important implications for addressing a crucial issue in drug addiction research. That is, to what extent “the risk factors for the use or misuse motivational enhancement therapy: uses benefits techniques of a particular class of psychoactive substances are specific to that class or are nonspecific in that they predispose the individual to the use or misuse of a wide range of such compounds” (Kendler et al. 2003, p. 687). Studies in twins (Tsuang et al. 1998; Kendler et al. 2003) have shown that the substance specificity of drug abuse is almost entirely due to environmental influences but very little is known about their nature (Zinberg 1984). Thus, it is remarkable that translational studies in humans have shown that the setting can influence drug reward in a manner similar to that observed in the rat.
Examples of these drugs are alcohol, benzodiazepines (e.g., lorazepam and diazepam), cannabis and opioids (e.g., heroin) [149]. Two of the most common combinations are cocaine in conjunction with alcohol and opioids/heroin (also known as ‘speedball’) [1,24], and therefore will be given special standout.
